1. Is the door width in the room wide enough for a wheelchair?
2. Is there an automatic door opener?
3. What type of door handle is there?
4. Is the door easy to open?
5. Is there a lowered peep hole?
6. Is there room for the door to swing open beside the wheelchair?
7. How many beds are in the room?
8. Is the frame open under the bed?
9. What is the size of the beds?
10. What is the height of the beds?
11. Is there room between the beds for a wheelchair?
12. Is the height of the table in the room accessible for a wheelchair?
13. Are the light switches lowered?
14. Are the lamps touch on and off?
15. Is the thermostat accessible and lowered?

3. Is the parking covered?
4. Is the location to the front door good?
5. Are the stalls extra wide?
6. Are there curb cuts at the entrance?
7. Is the curb/ramp to and from the parking good?
8. Are the entrance doors to the hotel automatic?
9. Is the front desk lowered?
10. Are the buttons on the outside/inside of any elevator lowered?
11. Is the flooring carpeted?
12. Is there a lowered public telephone?
13. Are the public restrooms accessible?
14. Is there a double door entry into the public washrooms?
15. Is there a raised toilet in the public washroom?
16. Are both male and female accessible public washrooms available?
17. If there a dining room, is it accessible?
18. Are the dining tables raised to allow access with a wheelchair?
